Job Description for Innovations Waiver Program (IW-MR/DD)

Community Living and Supports QP

Education: Bachelor Degree

Duties: Innovations Waiver (IW) Services Qualified Professional (QP) supervises staff that are providing community support services to clients. QP will meet with staff and families monthly to evaluate services provided, process any barriers to service and facilitate ongoing trainings.

Responsibilities:

• To assist in the development and/or maintenance of skills, behavior, and responsibilities as outlined in the plan of care needed to function successfully, with the greatest potential degree of self-determination and independence, in the community.
• To provide training with activities of daily living skills (e.g. bathing, dressing, meal preparation, shopping, laundry, transportation to access other service provision, resource management).
• To assist in the development and/or maintenance of appropriate training and habilitation by providing monitoring and support of the consumer in a home and/or community setting/environment.
• To assist in acquiring and maintaining the adaptive and coping skills that are required to prevent relapse and/or de-compensation including the development and maintenance of motivation and skills that will increase the ability to access resources needed to remain in the least restrictive environment and to be successful in the community.
• To provide the appropriate documentation for service delivery; including service plans and service notes specified by Medicaid and Structured Family Interventions, LLC standards.

Requirements:

• Be available to supervise community living and support service delivery.
• Provide at least 1 hour of direct supervision each month to staff.
• Document monthly supervision according to Medicaid and Structured Family Interventions, LLC standards.
• Review staff documentation on a monthly basis at the Structured Family Interventions, LLC office.

Qualifications:

• Must be at least 21 years old and able to read and write
• Demonstrate knowledge, skills and abilities required by the population served
• Exhibit core skills including: technical knowledge, cultural awareness, analytical skills, decision-making, interpersonal skills, communication skills and clinical skills
• TB skin test
• First Aid and CPR Training, NCI Interventions core training, infectious diseases blood bourn pathogens/OSHA certificate
• Client's rights training
• No substantiated findings of abuse or neglect listed on the North Carolina Healthcare Personnel Registry
• Disclosure of any criminal conviction
• A valid driver's license if transporting clients
• No history of alcohol or drug abuse in the two years previous to employment

Job Type: Part-time

Pay: $20.00 - $21.00 per hour

Expected hours: 5 – 10 per week

Work Location: Hybrid remote in Durham, NC
